GENERAL INFO. (INSURANCE)
 

These guidelines were developed from the Slo-Pitch National Official Rules Book; the Citywide Sports League Playing Rules; and various other sources. This information was composed to educate all players, managers/coaches, umpires, and non-playing persons. This may seem lengthy, but is important (if you want to understand properly).

In addition to this information, please refer to the 2012 Citywide Sports' Slo-Pitch Playing Rules.  A participant is eligible to file an insurance claim, provided THEY MEET ALL OF THE FOLLOWING CONDITIONS:

  • The player's complete personal information (first name; last name; address; phone number; birthdate) MUST BE ON BOTH THEIR TEAM'S 2011 SPN TEAM INSURANCE FORM COPY AND THE COPY SUBMITTED INTO CITYWIDE SPORTS
  • The player's complete personal information MUST BE CORRECT AND TRUE - AS FALSE, MISSING AND MISLEADING INFORMATION WILL NULLIFY ANY CLAIM
  • Any claimant found to be under the influence of ANY OF THE FOLLOWING: narcotics; illegal drugs; alcohol; AUTOMATICALLY NULLIFIES A CLAIM
  • All players must meet all playing eligibility requirements, as set forth by SPN and Citywide Sports during the 2011 playing season
  • All registered players playing in Citywide Sports MUST HAVE THEIR OWN FULL MEDICAL AND DENTAL INSURANCE, AND LOSS OF INCOME INSURANCE (in case of any possible injury or incident which may take place during the course of the slo-pitch playing season)
  • The person who is making a claim PAYS ALL REQUIRED INSURANCE DEDUCTIBLE (S) AND OTHER ASSOCIATED COSTS, RELATING TO THEIR CLAIM.

PROPER STEPS FOR FILING AN INSURANCE CLAIM (ASSUMING ALL OF THE ABOVE-MENTIONED CONDITIONS HAVE BEEN FULFILLED IN ITS ENTIRETY:

  • The injured player's team manager/captain must contact Citywide Sports, and report an injury NO LATER THAN 24 HOURS FOLLOWING THE TIME OF INCIDENT
  • The injured player's team manager/captain (and any witnesses at the time and place of accident) must assist the umpire (who was present) with completing their incident report immediately following the game

IT IS THE TEAM MANAGER/CAPTAIN'S RESPONSIBILITY TO ENSURE THAT THIS HAS BEEN FOLLOWED UP WITH, WITHIN A REASONABLE AMOUNT OF TIME.  THE LEAGUE WILL ALSO FOLLOW UP WITH THE UMPIRE AND THE TEAM CAPTAIN/MANAGER, TO ENSURE THAT THIS PROCESS HAS BEEN COMPLETED.

When this information has been completed in its entirety, the umpire will send a copy to the League and the Umpires Association.  The League will then send a copy to: the National and Provincial Slo-Pitch National offices, the team manager/contact; and the injured player(s).

BECAUSE TIME IS OF THE ESSENCE, IT IS IMPORTANT THAT ALL PARTIES UNDERSTAND AND FOLLOW THIS PROCESS PROPERLY.
